here is my custom form

i have create a plugin and upload file with <input type="file" name="myfile">. but when i click on it i can't upload file from media library. can any one help.

In order to integrate upload to Media Library functionality, please follow following points:
1) Check if path of uploads folder is properly set without leading "/"
2) Check Write permissions to uploads folder

thanks but i want to attached file from media library currently i can't attached from there. i can upload from local directories but i do not need this. Files upload and save successfully no issue about upload.
