sterndata on "Separate Image Not Showing"

Hello, I wanna ask about some problem in my site running WordPress 4.5, the problem is many of my files can't be open whenever I'm accessing wp-content/uploads/2016/04/filename.jpg

Is this because the new version or some issue on site Permissions ..

even siteurl.com/wp-admin/upload.php result page can't be found
