bolter on "[Plugin: List Custom Taxonomy Widget] Bug: Settings gone when widget is in use twice"

Hello,
thanks for the great plugin.

One little bug, i have discovered recently.
If I add the widget to one sidebar, there is no problem. But if i put it into another sidebar, the settings are gone from the widget. They dont show up in neither of the two. (Supposedly the same if you put it into a third sidebar, and so on).
If I drag it off one of the sidebars, and refresh the page, then the settings retu.
