That is damn strange.

I'm going to go walk through the code & see if I can come up with anything that could explain this ... it'll take a little while.

I hope this isn't some weird IIS thing or Windows permission thing. I can't imagine how it could be, but you're the first person I've seen on azure, so just speculating. You might try tuing off as many other wordpress plugins as you can. Now and then some weird conflict with some plugin will cause strange behavior .. unlikely, but it could be. Tu off your caching plugin, anything that has to do with URL changes or rewriting or permalinks, anything to do with ajax, anything to do with error-catching, etc.

Another option is tu on WP_DEBUG --- however, you have to make sure the output goes to a file and not to the screen. If it's set to go to the screen then it will break the ajax update (after you press submit).

You might try that stuff and I'll update in a bit as to whether I've found anything or not...

ps - I'd still like you to upload a new file that is NOT a pdf & try again. In other words, upload a .jpg file into Download Monitor and make sure it has a filename you have never used. You can try replacing the download id in your existing test page shortcode ... I'd actually prefer a whole new contact form & new wordpress page, but it's probably ok to just replace the existing id with the id associated with the .jpg.
