1. On multisite is it possible to only cache the main site?

No really. On multisite it is only possible to activate plugin in network mode, so all settings are in the network admin. They cannot be modified for each subsite.

2. Does the cache add or change the database in anyway.

Yes, it adds own table in database and also save settings in existing tables (but its not changing them).

3. Does it work on a windows server?

It was testes with IIS 5+ according to FAQ - from other threads on forum it looks like configuring w3 total cache will be more harder than for apache and nginx.

We are aware of no incompatibilities with apache 1.3+, IIS 5+ or litespeed 4.0.2+. If there's a web server you feel we should be actively testing (e.g. lighttpd), we're interested in hearing.
