Some of the fields don't react as required fields.
they don't tu red if not filled out and the form sends.
they also do not disappear on success message!.

Looks like you're no longer using the plugin...

We had to use normal contact form, because your plugin stopped working. I tested it on a different theme and it was fine, so I guess it is some kind of script conflict going on there.
The problem was with salient theme from theme forest, fyi
