How to improve my conversion rate?

I'm looking for some suggestions on how I can improve my conversion rate.

The page in question is currently converting at less than 2%, and I'm hoping that some of you might be able to provide feedback to help get it over 2%.

I don't want to have to redevelop the entire page, so I am hoping there are some small minor improvements I could make.
