digmasolution on "[Plugin: Google Forms] Error: Ask for log in to see the form"

hi.. I tried to use this plugins many times. and it always end up by saying SIGN IN TO CONTINUE TO FORMS
no matter what I did. whether I put the code within the page, or I link directly to the google form when setting up the menu
