Checkout page is empty~Short code won't work

I don't want the prices showing at all. I simply want a request quote form at my check-out page.

I tried to use the short code and then linked the check-out page to the quote page I created with the short-code inserted, but the page is empty still. what could be wrong?

shortcode is only for normal checkout

Try this:
1- go to woo-rfq setting and make checkout option to be normal
2- "select rfq page" then pick the page you created . in your case "Quote".

if you choose RFQ then you don't need the shortcode or the page. your checkout only accepts request for quote and none of the prices show.

Step 2 above is optional really. you just need to change your checkout mode to "normal"

Thanks gplsaver. I have so stressed myself over this. I have it set now.
However, the "Submit your request for Quote" button is dormant. When you click it nothing happens.
